Healthcare clowning is a unique art form derived from traditions of European theatre and street clowning, where the central quality of performance is the connection between the clown and the audience.

Clowndoctors value, validate, and amplify a young person’s unique voice, unique humour, and unique imagination at times when they might be feeling powerless, anxious, lonely or bored. Clowndoctors do not arrive with a script; they arrive ready to respond to each child’s mood, needs, and imagination.

As they put it: “Underneath our foolish exterior, Clowndoctors are experts in interaction. We are available to everyone in the hospital, and we create invitations to interact and connect wherever we go. Our visits are improvised with each young person, giving them agency and responding to how they are feeling in the moment.”

A Moment That Changed the Room

One afternoon, staff asked the Clowndoctors to visit a young patient who had been struggling emotionally for several days. The atmosphere in the room was tense; the child was withdrawn, and her father was visibly exhausted.

As the Clowndoctors entered—tentative, curious, and deliberately “low‑status”—they began a gentle, improvised interaction, following the child’s cues. Within minutes, the mood in the room shifted. A small smile appeared. Then a giggle. Soon, the child and her father were laughing together for the first time in days.

A doctor who witnessed the moment later reflected: “I couldn’t believe the difference in my mood and stress levels in such a short period of time. I’m so grateful to the Clowndoctors. It was the first time I had seen this patient and her father laughing and smiling.”

This is the essence of healthcare clowning: not distraction for distraction’s sake, but emotional connection that helps children feel seen, heard, and in control.

Together with the young person, the Clowndoctors co-create an encounter where the young person is always the expert of their experience and is always in charge of their Clowndoctors.  In Children’s Hospitals this can be as delicate as a lullaby composed with a baby’s gurgles, as chaotic as a slapstick battle with uncooperative hospital furniture, as poetic as an epic adventure acted out by fingertips on a bed-frame, or as dramatic as a soap opera, complete with cliffhanger to be resolved in next week’s visit.

Supporting Children, Families and Staff

Families consistently describe Clowndoctors’ visits as a highlight of the week—something that breaks up long days, lifts spirits, and creates positive memories in difficult circumstances.

One parent shared: “Uplifting and something to look forward to each week. Personalised to the child. Made them feel special.”

Another said: “Knowing our daughter is stuck in hospital, we know she looks forward to these visits. The Clowndoctors really include her, which brings us joy.”

Staff also report that Clowndoctors help build trust, reduce anxiety, and support smoother clinical interactions.

A senior charge nurse explained: “They enhance what we’re trying to do… They interact with children who won’t interact with us. Even after they’ve gone, the children are happy, and we enjoy a much better relationship with them.”

A Creative Intervention with Measurable Impact

Delivering over 8,500 individual interactions across hospitals, hospices and ASN schools and specialist units across Scotland. Their work contributes to:
